Athletic Training and Insurance Forms

The following forms must be filled out thoroughly and accurately before a student-athlete will be cleared by the Dominican University Athletic Training Staff for practice and/or competition. Forms must be completed and turned in to the Dominican University Sports Medicine office by August 1, 2009, for all Fall athletes and September 1, 2009, for all Winter and Spring athletes.
